Chad was born January 2, 1987 in Orlando, Florida, to father, Frederick D Smith Jr. (Mt Plymouth), and mother, Madeline Kendall (Orlando). Chad was engaged to Sherri Hawkes. He worked at Garden Vision in Mount Dora.
Chad is survived by his father, Frederick D. Smith Jr. (Mt. Plymouth); mother, Madeline D. Kendall (Orlando); fiancé Sherri Hawkes and daughter Raelynn Smith; grandparents, Frederick D. Smith Sr. (Orlando); Welda Kendall Sr. (Mayo); Charlotte and Elliot Moore (Mt. Plymouth); uncles, Todd Kendall (Clermont) and Brad Smith (Orlando); Aunts: Elicia Prine (Mayo), Welda Kendall Jr. (Mayo), Melissa Goltermann (Winter Garden), Michelle Jones (Winter Springs) and Candice Groves (Marietta, GA).
A memorial service will be held at 2:00pm on Sunday, April 13, 2014, at DeGusipe Funeral Home & Crematory, 905 Laurel Avenue, Sanford, Florida 32771. 407-322-5131
